Does Botox or any neuromodulator hurt? Can you exercise after Botox? What about makeup, washing your face, alcohol, or lying down?

If you’re thinking about getting Botox, you’ve probably heard a lot of different rules about what you can and can’t do afterward. The good news? Botox is a very convenient treatment with little to no downtime for most people. You don’t need to spend the rest of the day sitting perfectly still or worrying that one wrong move will ruin your results. There are just a few simple things to keep in mind after your treatment.

Here’s what I tell my patients about Botox aftercare.

Does Botox Hurt?

For most patients, Botox is very comfortable. The needles are extremely small, and each injection takes just a moment. Most people describe the sensation as a quick pinch or tiny sting. You may feel a little pressure during the injections, but treatment is usually over within just a few minutes. Afterward, you might notice some temporary:

  • Redness
  • Small bumps at the injection sites
  • Mild swelling
  • Tenderness
  • Bruising

The small bumps usually settle fairly quickly. Bruising, if it happens, can take a few days to fade. If you’re nervous about needles, tell your injector. We can take a little extra time to make sure you’re comfortable.

Is There Any Downtime After Botox?

Usually, no. That’s one of the reasons Botox is so popular. Most patients can leave their appointment and go right back to their day. You can go to work, run errands, meet a friend for lunch, or continue with your normal routine.

You may have a few tiny bumps or some redness immediately afterward, but these are usually easy to live with. If you have an important event coming up, however, I recommend giving yourself some extra time. Even though Botox has minimal downtime, you can occasionally bruise.

What Should I Avoid After Botox?

The biggest thing to remember is: Be gentle with your face.

You don’t need to avoid touching your face completely. Just avoid rubbing, massaging, or putting significant pressure on the areas where you were injected.

For the first few hours, I recommend:

  • Don’t massage the injection areas.
  • Don’t aggressively rub your face when applying skincare or makeup.
  • Avoid treatments that involve significant pressure on the treated areas.
  • Follow the specific instructions your injector gives you.

That’s really it. You don’t need to walk around afraid to move your face.

Can I Exercise After Botox?

Yes but give it a little time. I generally recommend waiting at least two hours before strenuous exercise.

So if you normally work out in the morning, you can simply work out before your Botox appointment or wait until later in the day. Walking and normal daily activity are fine. And if you have a favorite workout routine, there’s no need to give it up because you’ve had Botox. The American Academy of Dermatology also recommends waiting two hours before strenuous physical activity after botulinum toxin injections.

Can I Wear Makeup After Botox?

Yes. You don’t have to go makeup-free after Botox. You can generally apply makeup immediately after your treatment. Just remember to use a light touch. Instead of rubbing foundation or concealer into your skin, gently pat it on. The same goes for removing your makeup later that evening. Be gentle around the areas that were injected. The American Academy of Dermatology confirms that makeup can generally be applied immediately following botulinum toxin treatment as long as patients avoid rubbing the treated areas.

Can I Drink Alcohol Before or After Botox?

This is one of those questions where I like to keep things simple. Alcohol doesn’t directly interfere with how Botox works, but it may make bruising more likely. Because Botox involves small injections, some patients develop a little bruising. If you’re someone who bruises easily or you have an important event coming up it makes sense to skip alcohol around your appointment. If you have a Botox appointment tonight, this isn’t the night I’d plan a big cocktail hour. And remember to tell your doctor about any medications or supplements you’re taking, especially anything that can increase bruising. Don’t stop a prescribed medication without talking to the doctor who prescribed it.

Can I Lie Down After Botox?

You’ve probably heard, “Never lie down for four hours after Botox.” It’s one of the most common Botox aftercare rules patients hear but you don’t need to spend four hours sitting perfectly upright. Older aftercare instructions often recommended staying upright for four hours. Today, the more important advice is to avoid rubbing, massaging, or putting significant pressure on the treated areas.

If you’re tired after your appointment, normal reclining is fine. You can sleep normally that night. If you’re comfortable sleeping on your back, that’s a good way to minimize pressure on your face. Sleeping on your stomach isn’t ideal immediately after treatment because it can put pressure on the treated areas. If you naturally roll onto your stomach during the night, don’t panic just avoid deliberately pressing your face into the pillow. The bottom line: You don’t need to worry about staying perfectly upright. Just be gentle with your face and follow any specific instructions from your injector.

When Can I Wash My Face After Botox?

You can wash your face after Botox. You don’t need to go to bed with a dirty face just because you had injections. When washing your face, simply be gentle:

  1. Use your usual gentle cleanser.
  2. Use lukewarm water.
  3. Don’t scrub or massage the injection areas.
  4. Pat your face dry with a clean towel.
  5. Apply your moisturizer and sunscreen as usual.

The goal isn’t to avoid touching your face. It’s to avoid aggressive rubbing or pressure.

Can I Go to Work After Botox?

Absolutely. For most patients, Botox is easy to fit into a busy schedule. Many people have treatment during their lunch break and go right back to work afterward. You might have a little redness or a few small bumps where the injections were placed, but these usually settle quickly. If you’re worried about bruising before an important meeting or event, schedule your appointment a little earlier to give yourself some breathing room.

Can I Go Out to Dinner After Botox?

Yes. In fact, this is one of the reasons patients love Botox. You can generally go about your normal day after treatment. Just be mindful about alcohol if you’re trying to minimize bruising, and don’t spend the evening rubbing or massaging your face. Your friends probably won’t even know you had anything done. And that’s usually the goal.

When Will I See My Botox Results?

One thing to remember is that Botox doesn’t work immediately. You may begin noticing a difference within a few days, but the effect develops gradually. I generally recommend waiting about two weeks before judging your final result. So don’t leave your appointment, look in the mirror, and think: “Nothing happened!” That’s completely normal. Give it time.

What Does a Normal Botox Day Look Like?

Let’s make it really simple.

Before your appointment

Come in with clean skin if possible, and make sure your injector knows about your medications and supplements.

During your appointment

Treatment usually takes just a few minutes. You’ll feel a series of small pinches as the injections are placed.

Immediately afterward

You can generally return to your normal activities. You can go to work. You can walk around. You can gently apply makeup.

A few hours later

You can resume strenuous exercise after the recommended waiting period.

That evening

You can gently wash your face and follow your normal skincare routine.

Over the next few days

Be patient. Your Botox is gradually beginning to work.

Around two weeks

You should have a much better idea of your final result.

Botox Aftercare: The Short Version

If you only remember five things, remember these:

1. You don’t need downtime.
Most people can return to normal activities right away.

2. Be gentle with your face.
Don’t rub or massage the injection sites.

3. Wait at least two hours before strenuous exercise.
Your normal daily activities are fine.

4. Makeup and gentle face washing are okay.
Just don’t aggressively rub your skin.

5. Don’t panic.
Botox takes time to work. Give it several days, and evaluate your final result around two weeks.

The Bottom Line

Botox aftercare is much easier than many patients expect. You don’t need to cancel your entire day, avoid facial expressions, or sit perfectly still after your appointment. For most patients, the biggest rule is simply to treat your face gently for the first few hours. And if you ever have questions after your treatment, don’t rely on something you read online. Ask your injector. Your aftercare instructions may be different depending on where you were treated, how much was injected, and what other treatments you may have had. The goal of Botox isn’t to make you look like you’ve had Botox. It’s to help you look refreshed, rested, and naturally like yourself.
